IDP Trial Review Form:  Available in MS-Word.

To Download the IDP Trial Review Form:  Right-Click on File Link above, then click "Save Target As", then choose the Directory on your Personal Computer (PC) that you wish to save the file to and Click "Save".

To Complete form:  Open file from the location on your PC.  Complete items 1 through 6, then save the file as a different name than the original blank form.  (Example:  IDP Trial Review 02F12345 or Smith Trial Review, etc.)

To Attach File to E-Mail:  Once you have opened a New Mail Window and addressed it to:  cazevedo@sacbar.org, click "Insert", "File" then locate the completed form from the location where you saved it on your PC, then click OK.  The file should now be attached to your e-mail.  Some E-Mail programs have slight variations, but this procedure should work in most instances.
